Ohio Elevator License

 Individuals who are currently in the business of providing conveyance services must apply for an elevator mechanic's license by submitting an other training application (the majority of us) or legacy application (not us in a majority of instances).


    1. The other training application is for individuals who wish to apply for a license based on passing the mechanic examination of a nationally recognized training program or completing a registered apprenticeship program (the majority of us). 


    2. The legacy application is for individuals who wish to apply for a license based on having at least three years of unsupervised experience immediately preceding November 1, 2024. 


Applications for an elevator mechanic's license are now open. Applicants must submit all of the following information and/or items at the State of Ohio's eLicense Portal:

Other Training Requirements (again, the majority of us)

  • Completed application
  • Proof of having passed the examination of a nationally recognized elevator mechanic training program OR proof of having completed a registered apprenticeship program for elevator mechanics (e.g., certificate of completion or other evidence)
  • Criminal records check
  • $250.00 application/licensing fee
  • $3.50 eLicense transaction fee

Legacy Requirements

  • Completed application
  • Proof of having worked as an elevator construction, maintenance, or repair person, without direct and immediate supervision for an elevator contractor authorized to do business in Ohio for a period of not less than three years immediately preceding November 1, 2024
    • Employment history
    • Tax documents (three years' worth)
    • Experience verification form
      • If you worked for an elevator contractor authorized to do business in Ohio, you must provide an experience verification form from each employer.
      • If you worked as a sole proprietor, you must provide an experience verification form (sole proprietor) from at least one party who hired you (or your company) for each of the three years.
  • Criminal records check
  • $250.00 application/licensing fee
  • $3.50 eLicense transaction fee


The legacy application will no longer be available starting November 1, 2025. Again, the majority of individuals in our Locals should be selecting the "other training" application, which will continue to be available.

Additionally, attached are guides for the legacy application, other training application, creating an eLicense account and the experience verification form, if needed. also attached are instructions from the State regarding the Backgrounds requirements for In-State and Out-Of-State residents. 


NEW INFO CONCERNING BACKGROUND CHECKS (DIRECTLY FROM THE STATE):
If the applicant is a resident of Ohio, they get the documents titled “WebCheck Request Form” and the “Criminal Records Check Instructions Ohio Residents.”  If they are not Ohio residents, they get “Fast Fingers Request Form” and “Criminal Records Check Instructions Out of State.”  In either case, the Ohio Revised Code number required (sometimes referred to as the “reason code”) is the same, 121.08.
